Funny you ask, I did a DVD today.
Used MakeMKV to transfer it and wrap it in MKV container. Then ran it through DoM.
It's currently encoding, but everything looked fine, so I'm sure it's fine. Done it plenty of times before.

It does sound like you didn't get the DVD transferred properly. Did you just copy the file from the DVD to your harddrive or did you use some kind of 'ripping' software?
Just copying usually doesn't work.
